FSBP Self and Family enrollees pay 54% more in 2027 ($134.38 more per paycheck). Self Plus One rises 66% and Self Only 43%. The Foreign Service Benefit Plan's increase is the second-largest in FEHB in every enrollment type, behind only MHBP Standard.

The figures below come from OPM's 2026 and 2027 FEHB rate files. AFSPA's website still lists 2026 prices; OPM's 2027 rate file is the 2027 source. Open Season runs November 9 to December 14, 2026.

Enrollment type 2026 biweekly 2027 biweekly Change per paycheck Change
Self Only $100.36 $143.75 +$43.39 +43%
Self Plus One $257.96 $429.35 +$171.39 +66%
Self and Family $248.27 $382.65 +$134.38 +54%

Over 26 pay periods, the Self and Family increase comes to about $3,494 for the year.

Retirees pay monthly. A Foreign Service retiree with Self and Family pays $829.07 a month in 2027, up $291.14. Self Only goes from $217.45 to $311.46 a month, and Self Plus One from $558.91 to $930.26.

Why the Enrollee Share Jumped More Than the Premium

The plan's full premium rose 23.5%. The enrollee share rose faster because the government's share is capped.

In 2026 the government covered about 75% of FSBP's premium for Self Only and Self and Family; Self Plus One was already at the maximum government contribution, about 73.4%. In 2027 all three tiers reach the government's maximum contribution, so the enrollee absorbs the rest. The government's contribution still went up in dollars (from $301.09 to $352.04 biweekly for Self Only), but its share of the premium fell to about 71% for Self Only, 64% for Self Plus One and 69% for Self and Family.

FSBP's full premium increase, 23.5%, is the second-highest in FEHB.

Two-Person Households: Check Self and Family

In 2027 FSBP's Self Plus One premium ($429.35) is higher than its Self and Family premium ($382.65). A two-person FSBP household pays $46.70 less per paycheck in Self and Family than in Self Plus One in 2027, about $1,214 over a year.

AFSPA confirms a two-person household can pick Self and Family instead of Self Plus One. If you and one family member are enrolled in Self Plus One, this is a change you can make during Open Season. For more on how the two tiers compare, see Self Plus One vs. Self and Family.

What Changes in the Plan Itself

FSBP (plan code 40, brochure RI 72-001) is a fee-for-service FEHB plan with a PPO network. FSBP has one option, High Option: 401 Self Only, 403 Self Plus One, 402 Self and Family. There is no HDHP.

  • Deductible: FSBP's deductible doesn't change for 2027: $300 Self Only, $600 for two or more in network.
  • Out-of-pocket maximum: FSBP's family in-network out-of-pocket cap rises from $8,000 to $12,000; the Self Only cap stays $6,000.

Who Can Enroll in FSBP

FSBP is not open to every federal employee. AFSPA says FSBP is for the Foreign Service and for executive-branch employees whose work involves foreign or intelligence affairs, in the U.S. or overseas.

AFSPA's list of eligible agencies is long: State, USAID, much of DoD and DHS, the intelligence agencies, FBI, DEA and others. Check the list before assuming you're in or out.

For retirees who are otherwise eligible to keep FEHB, AFSPA's plan-membership test covers Foreign Service annuitants and civilian retirees who had FSBP for at least one year while working. Foreign Service annuitants change plans through State's Retirement Division.

When a Switch Takes Effect
