Correction box, October 21, 2026. A claim in one r/FedEmployees thread says that in 2027 "every plan" in FEHB moves GLP-1 drugs like Wegovy and Zepbound to a 50% cost share, costing members $1,000 to $1,200 a month. No OPM or carrier document we could find says that. OPM's own 2027 plan-benefits file shows coinsurance on covered brand tiers from 25% to 60% depending on the plan and the tier, and some plans use flat copays. 50% is common on some tiers of some plans. It is not a rule for every plan.

If you take a GLP-1 and you are choosing a plan for 2027, the number you need is your own plan's cost for your drug, and it lives in your carrier's 2027 formulary. Here is where the 50% figure came from, what the documents show, and how to price your drug before Open Season ends on December 14.

Where the 50 Percent Figure Came From

The claim started in an r/FedEmployees thread about Compass Rose replacing MHBP for 2027. The original poster was reading OPM's 2027 plan-benefits spreadsheet. One commenter then used Compass Rose's own drug-pricing tool and reported "~$400 a month" for Ozempic or Mounjaro and "$1000-$1200 a month" for Zepbound or Wegovy.

A reply widened it: "This is for every plan now basically. I priced with the tool for samba, FSBP, etc and they are all 50 percent cost share." That reply cites no OPM carrier letter and no brochure. Even in the same thread, another member reported Zepbound on Compass Rose High at "about $400/mo without the coupon."

What OPM's 2027 Benefits File Shows

OPM publishes a 2027 FEHB Plan Benefits spreadsheet. The current version, revised October 15, 2026, lists every plan's in-network cost share by drug tier, from Tier 0 to Tier 6. We downloaded it and read the rows for the nationwide plans. The file does not name GLP-1 drugs, and tier names (preferred brand, non-preferred, specialty) vary by plan, so read this as each plan's tier structure, not as a GLP-1 price.

Source: OPM, 2027 FEHB Plan Benefits public use file (2027-fehb-plan-benefits_101526.xlsx), in-network rows, columns Tier 2 through Tier 5, read October 21, 2026. FedTools selected the nationwide plans and condensed Tiers 4 and 5 where they match; the file states no per-fill maximums, which sit in each brochure. For Compass Rose, the file's raw Tier 4 and 5 entries conflict with the carrier's own 2027 comparison page, which uses percentages with caps for specialty drugs (covered below). We could not validate SAMBA's entries against its own material either, so we do not show those cells, and they should not be used to estimate what you pay.

There is no single 2027 percentage. Coinsurance on the covered brand tiers ranges from 25% to 60%, and some plans, like Compass Rose High, use copays. 50% does appear: on Aetna's HDHP across its brand tiers, on GEHA Elevate, and on the third tier of several plans. If your GLP-1 lands on one of those tiers, 50% may be your number. That depends on your plan's formulary, which the file does not show.

Every nationwide option in the file lists "Chronic Disease Management: Obesity" as covered.

Compass Rose, the Plan the Claim Started With

Compass Rose's own 2027 comparison page fills in what the OPM file leaves out: the caps. On Compass Rose Standard, a preferred brand at retail is 40% up to a maximum of $400 for a 30-day fill. Specialty fills are 50% up to a maximum of $1,500 for a preferred brand, and non-formulary brands are 100%.

Compass Rose High uses a $75 copay for preferred brands and 25% up to a maximum of $300 for preferred specialty drugs.

So the only 50% on Compass Rose Standard's 2027 drug table is specialty pharmacy, capped at $1,500 for a preferred brand. The commenter's "about $400" for Ozempic or Mounjaro fits the Standard Option's $400 cap on a preferred-brand retail fill, if that is where the drug sits. Compass Rose's page does not say which tier Wegovy or Zepbound falls in. That is in the OptumRx formulary.

Why $1,000 a Month Does Not Add Up at 50%

The arithmetic does not support the claim. Novo Nordisk lowers the U.S. list price of Wegovy and Ozempic to $675 a month on January 1, 2027. TrumpRx shows Zepbound's original price as $1,087 a month and Wegovy pens' as $1,349.

Drug List price used 50% share
Wegovy or Ozempic, from January 1, 2027 $675 $337.50
Wegovy pens, TrumpRx original price $1,349 $674.50
Zepbound, TrumpRx original price $1,087 $543.50

FedTools calculation: 50% of each list price as published by Novo Nordisk (February 24, 2026 release) and TrumpRx (read October 21, 2026). Plans may not price off list, so treat this as a rough check, not your cost.

A charge of $1,000 to $1,200 a month lines up with paying roughly full list price. That can reflect an unmet deductible, an uncovered drug or another coverage restriction; the price alone does not identify the reason. It does not line up with a 50% share of any of these drugs.

Novo says its list-price cut is aimed at people "whose out-of-pocket costs are linked to list price," including high-deductible and coinsurance members. If your plan charges a percentage, a lower list price should lower your share. Novo's self-pay prices do not change with it.

What OPM Did Set for 2027

OPM's 2027 rules set coverage conditions, and we found no percentage in them. According to FedSmith's account of OPM's 2027 Open Season highlights, FEHB and PSHB plans keep covering at least one GLP-1 anti-obesity drug, and all carriers will require documented participation in intensive behavioral therapy before and during treatment. That therapy requirement applies to weight-loss use, not to diabetes prescriptions.

Diabetes drugs run on their own track. FEP Blue's 2027 summary lists "prescription copays and cost share maximums across brand name, specialty and diabetes-related drugs" as a 2027 change, without amounts. MHBP's 2026 page says members approved for anti-obesity medication must enroll in its weight-management program before coverage; its 2027 GLP-1 cost share has not been published.

How to Price Your Own GLP-1 Before Open Season Ends

  1. Find your drug's 2027 tier. Use your carrier's 2027 drug tool. FEP Blue's Drug Cost Tool shows 2027 prices starting November 6, 2026. Compass Rose prices through OptumRx.
  2. Read the cap, not just the percentage. A 40% share capped at $400 per fill is a very different year than an uncapped 50%.
  3. Check the program rules. According to FedSmith's account of OPM's 2027 materials, weight-loss use will need behavioral therapy documentation. Ask whether your current prescription carries over.
  4. Compare to cash. NovoCare lists Wegovy pens at $349 a month through 2026 ($199 for the first two months for new patients), and TrumpRx lists Zepbound at $299 a month. Cash payments may not count toward your plan's deductible or out-of-pocket maximum.
  5. Add it to the plan math. A drug you fill every month can matter more than the premium gap between two plans.

Open Season runs November 9 through December 14, 2026.

Compare Plans With Your Drug Cost In

The FEHB Plan Comparison tool puts two or three plans head to head on total annual cost, premium plus expected out-of-pocket spending, and shows the spend level where the cheaper plan flips. The tool models in-network charges using a simplified cost-sharing assumption, so do not enter a personal copay as a total charge. Compare your carrier's annual drug-cost estimate for each plan separately.

Frequently Asked Questions

Is every FEHB plan charging 50% for GLP-1 drugs in 2027?

No OPM or carrier document we found supports that claim. The figure came from Reddit comments about Compass Rose. OPM's 2027 benefits file shows coinsurance on covered brand tiers from 25% to 60% depending on the plan and tier, and Compass Rose High uses a $75 copay on its second tier.

Did OPM set a GLP-1 cost-share percentage for 2027?

Not in the OPM material we could read. According to FedSmith's account of OPM's 2027 highlights, plans keep covering at least one GLP-1 weight-loss drug and will require intensive behavioral therapy. Each carrier sets its own tier and cost share.

Why would Wegovy cost $1,000 or more a month on an FEHB plan?

That is roughly the full list price. A high quoted price may reflect an unmet deductible, an uncovered drug or another coverage restriction; the price alone does not identify the reason. Half of Wegovy's January 2027 list price is $337.50, and half of Zepbound's $1,087 original price on TrumpRx is about $543.

Does the Wegovy price cut lower what I pay in 2027?

It can. Novo Nordisk drops the list price of Wegovy and Ozempic to $675 a month on January 1, 2027 and says the cut targets coinsurance and high-deductible members. Its self-pay prices do not change.

Is Ozempic for diabetes priced the same as Wegovy for weight loss?

Not necessarily. Plans handle diabetes drugs separately, and FEP Blue lists diabetes-related drug cost shares as their own 2027 change. According to FedSmith's account of OPM's highlights, the behavioral therapy requirement applies to weight-loss use.

How do I check my real 2027 GLP-1 cost before Open Season ends?

Use your carrier's 2027 drug tool. FEP Blue's goes live November 6, 2026, and Compass Rose prices through OptumRx. Compare that cost to the cash price before you enroll.
